An investigation has been launched into the freak accident that killed 3-year-old Wynter Larkin in Philadelphia's "Brewerytown" section on Saturday.

The heartbreak occurred right outside Rita's Water Ice at 29th Street and Girard Avenue. That's where crime scene investigators were working after the young girl was crushed by a 2,000 pound security door that came to rest on the sidewalk.

The community is still stunned and looking for answers.

“I’ll never forget it, little baby laying down there,” said witness Gregory Boyd.

Boyd returned to the scene on Monday with his own daughter. He witnessed the rescue efforts as onlookers frantically tried to save little Wynter Larkin of Yeadon, Pa.

While neighbors and shop owners struggle to comprehend the loss, their attention has switched to the cause, the contractor who installed it, and the size of the lag bolts holding the gate.

The police and Licenses and Inspections are still investigating all of those aspects.

“The person that put it in knows it supposed to be 6-8 inches, whatever the case was, it was only two and half inches,” said Boyd.

Juliet Askew runs a daycare a few shops down. She says every security gate needs to be looked at in the wake of this accident.

“Make sure all of them are safe now, properly installed,” said Ashkew.

Fox 29’s  Dave Schratwieser spoke with L &I Commissioner Carlton Williams on Monday. He will not comment on weight of the door, size or strength of the lag bolts that held the gate up. L & I says there were no complaints about the gate, and says the upkeep of the gate is the property owners' responsibility.
